BOTOX® Cosmetic: An Overview

BOTOX® Cosmetic is an FDA-approved neuromodulator made from a purified form of botulinum toxin type A. When injected in very small, controlled amounts, it temporarily relaxes specific facial muscles that create expression lines. These are the lines that form when you frown, raise your brows, or smile repeatedly over time. By softening muscle activity, BOTOX® allows the overlying skin to appear smoother while maintaining natural facial movement.

The treatment has been used safely in aesthetic medicine for decades and is one of the most studied cosmetic procedures worldwide. When performed conservatively and precisely, the goal is not to change how you look—but to reduce the intensity of certain lines so your face appears more rested and balanced. With training at Yale, Harvard–MIT, the University of Pennsylvania, and NewYork-Presbyterian’s Columbia and Cornell campuses, facial plastic surgeon Dr. Shomorony approaches BOTOX® and all non-surgical treatments with the same sophisticated understanding of facial anatomy and muscular balance. His philosophy centers on restrained, elegant, subtle results.

Expected Improvements Why Patients Choose BOTOX®

Facial lines caused by repeated expression—frowning, raising your brows, squinting—can gradually become more noticeable, even when your face is at rest. For many patients, this creates a mismatch between how they feel and how they appear. BOTOX® Cosmetic is one of several neuromodulators used to address these dynamic lines. (In fact, the term “Botox” is often used colloquially to refer to neuromodulators in general, even though multiple FDA-approved options exist.) BOTOX® works by temporarily relaxing specific muscles responsible for expression-related creasing. When administered in small, precise amounts, it softens lines without affecting your ability to smile, emote, or maintain natural facial movement.

Patients may choose BOTOX® over other neuromodulators when a more controlled diffusion pattern is preferred, particularly in areas requiring targeted precision such as the glabellar complex. It also has the longest track record in aesthetic use, which can offer reassurance for first-time patients. Our patients also tend to choose BOTOX® in Greenwich, CT because it is:

  • Extensively studied and well established
  • Predictable in onset and duration
  • Appropriate for both preventative and established lines
  • Subtle when administered conservatively

Note that, unlike dermal fillers, BOTOX® does not add volume or alter facial structure. It works by modulating movement. The result is smoother skin both in motion and at rest, while preserving natural expression.

What to Expect For First-Time Patients

If you’ve never had BOTOX® before, it is normal to feel unsure about what the appointment will actually be like. Treatment is performed in our Greenwich office and typically takes 10–15 minutes.

No sedation is required. A very fine needle is used to place small amounts of product into targeted muscles. Most patients describe the sensation as a brief pinch lasting only a few seconds per injection.

Frequently Asked Questions BOTOX®

Is it painful?

Discomfort is minimal. The needle is extremely small, and injections are quick. Most patients do not require numbing.

Will I look different immediately?

No. Results develop gradually. You may notice subtle softening beginning around day 4–5, with full results visible at 10–14 days.

Will I look frozen or expressionless?

When administered properly, BOTOX® does not eliminate expression. It reduces excessive muscle contraction. You will still smile, raise your brows, and look like yourself, simply with softened lines.

After treatment, is there downtime?

There is no true downtime. Mild redness or tiny bumps at injection sites can occur but typically resolve within 20–30 minutes. Most patients return immediately to work, errands, or social plans.

How long does BOTOX® last?

Results generally last 3–4 months. With consistent maintenance, some patients find longevity improves over time.

Does BOTOX® prevent wrinkles?

By reducing repetitive muscle folding of the skin, early treatment can help slow the deepening of dynamic lines.

Can BOTOX® be combined with filler?

Yes. Neuromodulators address muscle movement, while fillers restore volume. They are often complementary treatments.
